AMPLIFY LIFE GROUP PROGRAM PACKAGE
Group coaching is customized to your needs. Meetings take place conveniently on the phone, so you can still organize your life while committing to your health.

What the Program Includes:
· Eight thematic group sessions depending on the group and their health challenges, concerns and goals
· Your own 3-ring binder of supportive articles & handouts
· Recipes, customized menu planning, and cooking tips
· Healthy living materials / samples
· Two 60 minute consultations/month (8 sessions total)
· Free admission to regular workshops or teleclasses during your program
· Unlimited email support between meetings (48 hour turnaround)
· Lifetime access to any E-books, client mailings and newsletters
· And a few surprises : )

Dates for Group Coaching: (Times are in Central Standard Time)
Eight Sessions over 4 months
Mondays: 7:00pm
Tuesdays: 10:00am, 11:30am, 7:00pm
Wednesdays: 7:00pm
Thursdays: 8:00pm
Please choose a time and register below. Email your time preference to mindy@amplifylife.net after you register.
If none of the times listed work for you, please email mindy@amplifylife.net to be placed on the wait-list. I will see what I can do. :)
Cost (Eight 60-minute coaching sessions):
If you'd like extra one-on-one support during your program, you may schedule a private phone session. Please note, these sessions and pricing are only available to those already enrolled in the program. Email Mindy at any time during the four months to schedule additional phone sessions.
Add-on Session Pricing:
30 minute private phone session $35
60 minute private phone session $65

*By signing up for the Program you are agreeing to pay for AND participate in the entire Program
*Payment can be made online via credit card or checking account. Payments are non-refundable
*Once you sign up, you will receive a welcome letter and your first package of "goodies" will be sent out!
